Human and Organizational Performance (HOP) topics - Error Traps

Recognizing error-likely situations and applying tools to decrease the likelihood of errors.

Hi, my name is Katrina Stone (HSE workforce safety specialist) and I have joined CSC to help integrate Human and Organizational Performance (HOP).

HOP Focuses on reducing the seriousness and frequency of human failures and resulting outcomes by "Making it easy to get it right, and harder to get it wrong".
Over the next few issues of the Safety Bulletin you will read about HOP topics that will help build your fluency in this area so we can work together to:

  • Improve the interaction between the individuals and critical systems
  • Recognize error-likely situations and apply tools to reduce the likelihood of error


Distraction: Anything that prevents a person from giving full attention to the task they are currently performing:

  • feeling pulled in too many directions
  • not in control of the situation
  • leaving a task in the middle forgetting the condition when returning to a task
  • making minor errors
  • forgetful
  • trouble paying attention to detail. low level of concentration on each task

What are some potential consequences of those errors?

 

Both fatigue and stress can increase distractibility. Recognizing distraction individually and in each other can reduce the likelihood of error and minimize potentially fatal consequences.
