Real Life Incident: Finger Crushed While Operating Crane Jib

Two crew members were tasked to secure the hose handling crane jib in its cradle support. One crew operated the crane while the other signaled. As the crane jib was lowered, the signaller attempted to manually align the wooden block over the jib cradle support.

Real Life Incident: Anchor Lost In Heavy Winds

A tanker in ballast dropped anchor and six shackles of chain in the early evening to await a berth for loading. Winds were force 4, but stronger winds were forecast so the main engines were kept on standby.

This time, the case illustrates what went wrong during an incident that took place in the Suez Canal with the aim to raise the discussion and perform an onboard risk assessment of the incident and the factors which led to it, based on vessel’s procedures.
